Your participation is voluntary and you may withdraw at any time without reprisal. You may leave any questions you are uncomfortable with blank. There are no anticipated risks to participating in this survey. Your participation is anonymous and your name will not be associated with your answers. Your responses will be kept completely confidential, to the extent permitted by law.

Although you may not directly benefit from your participation, others may ultimately benefit from the knowledge obtained in this study. This survey has been approved by the University of Michigan's Behavioral Sciences Institutional Review Board.
